  1. How we collect your personal information:  Kwok Daniel collects your personal information when you communicate with the attorneys, paralegals and/or staff at Kwok Daniel during in-person meetings, virtual meetings, intake, telephone conversations, and email correspondence.  Kwok Daniel may also view any public postings, public information, obtain information from third-parties, or through any other means according to the attorney’s discretion.
  2. What personal information we collect.  Kwok Daniel may collect your name, home address, phone numbers, email address, date of birth, social security number, driver’s license, your employment history, along with the name, address and phone number of any of your emergency contacts.  Kwok Daniel may also collect your auto insurance information, your personal health insurance, and whether you are a recipient of Medicaid, Medicare, Tricare, or any other governmental program providing health insurance and/or benefits.  Kwok Daniel may also collect information regarding your medical providers, and may obtain your medical records.  According to the attorney’s discretion, Kwok Daniel may obtain your employment records, W2s, tax returns, social security earnings, or any other economic data that may be relevant to a subject case.
  3. How we use your personal information.  Kwok Daniel Ltd. LLP uses your personal information so the firm may properly serve you, communicate with you, investigate your case, and prosecute your case.  Kwok Daniel may also use your personal information for internal purposes such as record keeping, compiling data, and research.
  4. How, why, and what personal information do we share with third parties.  Kwok Daniel will disclose your name, address, date of birth, driver’s license number, and the last four digits of your social security number to when making an insurance claim, and/or to prosecute your case.  Kwok Daniel will also disclose the medical providers, along with the medical records and bills, for the medical treatment related to your claim.  If necessary, Kwok Daniel will share any earnings information with respect to a lost wage/lost earning capacity claim.

    This information is typically provided to the auto insurance companies that may be responsible for paying your claim, and/or the attorneys that are hired by the insurance companies to defend a Defendant, or an insurance carrier.

    With the client’s consent, the information may be shared with any outside counsel retained by Kwok Daniel, Ltd LLP to prosecute and/or try your case.

  5. Mobile Opt In, SMS Content, and phone numbers collected for SMS communication purposes will not be shared with any third party and affiliates for marketing purposes.

      1. Message Frequency may vary depending on a number of variables such as (1) the type of communication; (2) the need for communication; (3) and the status of your case.   
  3. POTENTIAL FEES FOR SMS MESSAGING
      1. Please note that standard message and data rates may apply, depending on your carrier’s pricing plan.  These fees may vary if the message is sent domestically or internationally.
  4. OPT-IN METHOD
      1. You may opt-in to receive SMS messages from Kwok Daniel in the following ways:
        1. Verbally
        2. Sending a request via email.
  5. OPT-OUT METHOD
